Who is Eligible?

  • Residents of West Bengal

  • Must possess a valid Swasthya Sathi Smart Card

  • No income bar for eligibility

  • Only the woman of the family is considered the cardholder (as per the scheme)

Admission Process at Jeevan Rekha Hospital

Planned Admission:

  1. Visit the TPA Desk before the scheduled date

  2. Submit the smart card and medical documents

  3. Hospital will apply for pre-authorization

  4. On approval, admission is done without any deposit

Emergency Admission:

  1. Patient is admitted immediately

  2. Documents to be submitted to the TPA Desk within 24 hours

  3. Hospital applies for authorization on behalf of the patient

  4. After approval, treatment continues under the scheme

Cashless service means that patient will not have to spend any amount for taking the treatment and hospitalization. You do not have to worry about finances in case of a hospitalization, thus giving you peace of mind.

No, the OPD treatment of any type is not under the scheme. However, you are admitted after it, the cost of OPD will also be included as a part of Pre-hospitalization expanses.

Deposit the Swasthya Sathi Card within 24 hours from admission, else the cost will have to be borne by the Beneficiary.

Swasthya Sathi is a cashless scheme and treatment cost up to Rs 5,00,000 (Five Lakhs per annum per family) shall be paid to the Hospital from the smart card. NO PAYMENT IS TO BE MADE FOR TREATMENT UNDER SWASTHYA SATHI SCHEME by the beneficiary.

The benefit amount of INR 5 lakh is an annual amount that will be renewed every year.

Fingerprint verification is to prevent fraud and misuse of the smart card.

Your room entitlement will be General Ward or ICU (In case of critical care). You cannot upgrade your room to private rooms while admitted under this scheme.

In that case, the patient need to visit the nearest Swasthya Sathi office and get his name registered to get Cashless benefits under the Swasthya Sathi Scheme.

All bills in original and discharge certificate are to be left with the Swasthya Sathi Desk at Jeevan Rekha Hospital providing cashless treatment. The patient has to countersign all bills and fill the claim form and also leave the same with the hospital at the time of discharge. A copy of the bills, Discharge Summary & Reports can be carried by you for your records.

Fingerprint of any enrolled member of the family can be provided.

Yes, a request for authorization for Swasthya Sathi may be declined if, Inadequate/wrong information is provided and the Swasthya Sathi department is unable to get access to additional information. The ailment/disease for which hospitalization/surgery is required is not covered by the insurance policy. The person does not have adequate insured amount in the Swasthya Sathi account left to cover the hospitalization costs. This only means that cashless access is declined, AND IS IN NO WAY TO BE CONSTRUED AS DENIAL OF TREATMENT. The beneficiary must obtain the treatment as per his/her treating doctor’s advice & settle all the Hospital Bills at the time of discharge through Cash.
