Cashless service means that patient will not have to spend any amount for taking the treatment and hospitalization. 


You do not have to worry about finances in case of a hospitalization, thus giving you peace of mind.


Jeevan Rekha Hospital is 'A' Grade accredited by Swasthya Sathi. All advanced treatment modalities under Medical management and Surgical Intervention are available at our hospital.

No, the OPD treatment of any type is not under the scheme. However, you are admitted after it, the cost of OPD will also be included as a part of Pre-hospitalization expanses. 


If there is no hospitalization, then it has to be borne by the beneficiary. 


Deposit the Swasthya Sathi Card within 24 hours from admission, else the cost will have to be borne by the Beneficiary. 


  •  From the Discharge Slip, in case of hospitalization and the SMS received in the registered mobile.
  • Use the Swasthya Sathi Mobile App, available from Google Play Store, and search by putting your URN number.


Swasthya Sathi is a cashless scheme and treatment cost up to Rs 5,00,000 (Five Lakhs per annum per family) shall be paid to the Hospital from the smart card. NO PAYMENT IS TO BE MADE FOR TREATMENT UNDER SWASTHYA SATHI SCHEME by the beneficiary. 


No. The scheme is a floater scheme, which means that INR 5 lakh will be given collectively to the whole family. 


The benefit amount of INR 5 lakh is an annual amount that will be renewed every year. 


In case of Planned Admission:


  • Arrive at the Swasthya Sathi & TPA Desk on the 1st Floor two days prior for Registration with your Swasthya Sathi smart card.
  • Bring documents like - Swasthya Sathi smart card, Patient’s government issued Identity Card (like Voter’s ID card, Aadhar Card, Driving License), Doctor’s prescription, and relevant supporting reports.
  • Hospital will send the registration request to Swasthya Sathi for admission.
  • Once the registration is approved, you will be informed from Swasthya Sathi by call and you can get admitted.


In case of Emergency Admission:


If you are in need of urgent admission, get admitted in the hospital & then approach the 1st Floor Swasthya Sathi Desk within 24 hours of admission.


Non-Medical:

  • Swasthya Sathi Smart Card
  • Government Issued Photo Identity Proof of the patient (Voter Card, Aadhar Card, Driving license, Etc.)
  • Mobile Number


Medical:

  • Previous Medical Documents (if any)
  • Doctor’s Prescription
  • Past Investigation reports


Smart Card is essential to get the service. If card is not available, contact Swasthya Sathi desk.


Fingerprint verification is to prevent fraud and misuse of the smart card. 


Fingerprint of any enrolled member of the family can be provided. 


At Jeevan Rekha Hospital, Specialist Doctors are available across various facilities who attends to patients immediately after Planned or Emergency admissions like Heart Attack, Brain stroke, Burns, Accidents etc.


Your room entitlement will be General Ward or ICU (In case of critical care). You cannot upgrade your room to private rooms while admitted under this scheme.


In that case, the patient need to visit the nearest Swasthya Sathi office and get his name registered to get Cashless benefits under the Swasthya Sathi Scheme.


Yes, a request for authorization for Swasthya Sathi may be declined if,


  • Inadequate/wrong information is provided and the Swasthya Sathi department is unable to get access to additional information.
  • The ailment/disease for which hospitalization/surgery is required is not covered by the insurance policy.
  • The person does not have adequate insured amount in the Swasthya Sathi account left to cover the hospitalization costs.


This only means that cashless access is declined, AND IS IN NO WAY TO BE CONSTRUED AS DENIAL OF TREATMENT. The beneficiary must obtain the treatment as per his/her treating doctor’s advice & settle all the Hospital Bills at the time of discharge through Cash.


All bills in original and discharge certificate are to be left with the Swasthya Sathi Desk at Jeevan Rekha Hospital providing cashless treatment. The patient has to countersign all bills and fill the claim form and also leave the same with the hospital at the time of discharge.

A copy of the bills, Discharge Summary & Reports can be carried by you for your records.
