There is no cap in family size and parents from both the spouse are covered. Cards is issued preferably in the name of eldest women family member as Head of family
No contribution is to be made towards insurance premium or card cost on the part of the beneficiary as entire premium which including cost of smart cards is borne by the Government of West Bengal
The beneficiary avails the services in the form of Paperless, Cashless and entirely on the 64Kb Smart Card which contains digitally encrypted information printed in the card and distributed to the beneficiary during the time of enrolment. Smart card valid for lifetime and auto renewed in each year
Downloadable from Google Play Store and queries can be raised for information, grievance, etc. This also includes a one-touch feature which directly connects to the 24X7 Help Line.
There is interactive “May I Help You” option for general queries of the viewer. Common people can also view their eligibility, enrolment details in the web portal.

No, the OPD treatment of any type is not under the scheme. However, you are admitted after it, the cost of OPD will also be included as a part of Pre-hospitalization expanses.
If there is no hospitalization, then it has to be borne by the beneficiary.
Deposit the Swasthya Sathi Card within 24 hours from admission, else the cost will have to be borne by the Beneficiary.
Swasthya Sathi is a cashless scheme and treatment cost up to Rs 5,00,000 (Five Lakhs per annum per family) shall be paid to the Hospital from the smart card. NO PAYMENT IS TO BE MADE FOR TREATMENT UNDER SWASTHYA SATHI SCHEME by the beneficiary.

At Jeevan Rekha Hospital, Specialist Doctors are available across various facilities who attends to patients immediately after Planned or Emergency admissions like Heart Attack, Brain stroke, Burns, Accidents etc.
Your room entitlement will be General Ward or ICU (In case of critical care). You cannot upgrade your room to private rooms while admitted under this scheme.

This only means that cashless access is declined, AND IS IN NO WAY TO BE CONSTRUED AS DENIAL OF TREATMENT. The beneficiary must obtain the treatment as per his/her treating doctor’s advice & settle all the Hospital Bills at the time of discharge through Cash.
All bills in original and discharge certificate are to be left with the Swasthya Sathi Desk at Jeevan Rekha Hospital providing cashless treatment. The patient has to countersign all bills and fill the claim form and also leave the same with the hospital at the time of discharge.
A copy of the bills, Discharge Summary & Reports can be carried by you for your records.
